شادی کی گرانٹ
درج ذیل دستاویزات کے ساتھ ڈاؤن لوڈ کردہ درخواست فارم جمع کروائیں:
درخواست گزار/سرکاری ملازم کی CNIC کاپی
بیٹی کی شادی کا CNIC یا میٹرک کا رزلٹ کارڈ یا یونین کونسل سے پیدائش کا سرٹیفکیٹ
نکاح نامہ کی کاپی
حکومت کی پے سلپ B.F کٹوتی کے ثبوت کے ساتھ ملازم
اگر ریٹائرڈ حکومت ملازم پھر ریٹائرمنٹ آرڈرز/پی پی او کاپی
اگر حکومت ملازم مردہ ہے پھر ڈیتھ سرٹیفکیٹ کی کاپی
ٹاؤن کمیٹی/یونین کونسل سے جاری کردہ ڈیتھ سرٹیفیکیٹ
خاندان کے افراد کی فہرست (اگر سرکاری ملازم مر گیا ہے)
نکاح اندراج فیرسٹ/ نادرا رجسٹریشن سرٹیفکیٹ

متوفی/ غلط حکومت سرونٹ CNIC اور کمپیوٹرائزڈ پے سلپ

یونین کونسل سے جاری کردہ ڈیتھ سرٹیفکیٹ
اگر حکومت ملازم غلط ریٹائر ہو گیا پھر ڈی جی کی طرف سے انیلیڈیشن لیٹر جاری ہوا۔ صحت، پنجاب
درخواست دہندہ کا شناختی کارڈ اور تصویر
یونین کونسل ناظم سے تصدیق شدہ بیوہ سے دوبارہ شادی نہ کرنے کا سرٹیفکیٹ
سروس کے دوران ڈیتھ سرٹیفکیٹ ہیڈ آف ڈیپارٹمنٹ کی طرف سے جاری کیا جاتا ہے۔
اگر ریٹائرمنٹ کے بعد موت واقع ہوئی ہے تو ریٹائرمنٹ آرڈر یا پنشن بک کاپی
بیوہ کی جانب سے دوبارہ شادی نہ کرنے کا سرٹیفکیٹ اگر ملازم عورت ہے تو گزیٹڈ آفیسر سے تصدیق شدہ
نابالغ ہونے کی صورت میں یونین کونسل سے اس کا برتھ سرٹیفیکیٹ اور گارڈین کورٹ سے گارڈین سرٹیفیکیٹ
خاندان کے ارکان کی فہرست جس کا نام، تاریخ پیدائش، رشتہ (متوفی کے ساتھ)، مارشل اسٹیٹس اور پیشہ کے ساتھ ہیڈ آف ڈیپارٹمنٹ کی تصدیق کی گئی ہے اور خاندان کا کوئی دوسرا رکن نہیں ہے۔
نوٹ
تمام درخواست فارم مکمل طور پر پُر کیے جائیں ورنہ فارم مسترد کر دیا جائے گا۔

الوداعی گرانٹ حاصل کرنے کے لیے، درخواست فارم ڈاؤن لوڈ کریں اور درج ذیل دستاویزات کے ساتھ منسلک کریں:

Scholarship Grant
To apply for a grant, download the application form and submit it along with the following documents:
Student/Father/Mother CNIC copy
Latest Computerized Pay Slip Employees During Service Govt
Pension Book for Retired Government Servants
Affiliation letter of the student body with the Board/University
Latest Detailed Marks Certificate of Board/University with confirmation of Controller Examinations
Grant of marriage
Submit the downloaded application form along with the following documents:
CNIC copy of applicant/government employee
Daughter’s Marriage CNIC or Matriculation Result Card or Birth Certificate from Union Council
Copy of marriage certificate
Employee with Govt payslip B.F proof of deduction
If Retired Govt Employee then Retirement Orders/PPO Copy
If Government employee is dead then copy of death certificate
Death Certificate issued by Town Committee/Union Council
List of Family Members (if Government Servant is deceased)
Marriage Enrollment First/ NADRA Registration Certificate
Note
Submit the form to the parent department within 275 days.
Submit application form from parent department to B.F Board within 03 years.
Grant of marriage
Submit the downloaded application form along with the following documents:
CNIC copy of applicant/government employee
Daughter’s Marriage CNIC or Matriculation Result Card or Birth Certificate from Union Council
Copy of marriage certificate
Employee with Govt payslip B.F proof of deduction

Funeral Grant
To get the grant, download the application form and submit it along with the following documents.
CNIC copy of applicant/government employee
Death Certificate issued by Town Committee/Union Council
CNIC or Birth Certificate of deceased person and Death Certificate from Union Council
Note
Submit the form to the parent department within 275 days.
Submit application form from parent department to B.F Board within 03 years.
Monthly Grant
When applying for a grant, download the application form and submit it along with the following documents.
Deceased/ Incorrect Govt Servant CNIC and Computerized Pay Slip
B.F proof of deduction
Death Certificate issued by Union Council
If the govt employee retired wrongly then a cancellation letter was issued by the DG. Health, Punjab
Applicant’s Identity Card and Photograph
Certificate of Non-Remarriage of Widow attested by Union Council Moderator
In-service death certificate is issued by Head of Department.
Copy of Retirement Order or Pension Book if death occurred after retirement
Certificate of non-remarriage from the widow if the employee is female, attested by Gazetted Officer
In case of minor, his Birth Certificate from Union Council and Guardian Certificate from Guardian Court.
List of family members with name, date of birth, relationship (to deceased), martial status and occupation as verified by the Head of Department and no other family members.
Note
All application forms must be filled completely otherwise the form will be rejected.
Farewell Grant
To avail Farewell Grant, download the application form and attach the following documents:
CNIC copy of applicant/government employee
Last computerized payslip of Govt with proof of B.F deduction of employee
Death Certificate issued by Hospital/Union Council
Court issued affidavit if Govt. The employee has died in favor of the petitioner.
Notification of revised rates of all Part I and Part II grants
